Why AI Summaries Miss the Decision You Needed

Use a decision-ready meeting notes template to capture commitments, unresolved questions and conditions without turning tentative discussion into certainty.

TL;DR

  • The decisive sentence is usually short and unremarkable. "Fine, let us do annual" carries far less linguistic signal than the ten minutes of discussion around it.
  • The common failure is not omission but flattening: a conditional agreement loses its condition and is recorded as a commitment nobody made.
  • A usable decision record needs five separate fields, and the one most often missing is the condition attached to the agreement.
  • The template only works if the last minute of the call restates each decision aloud. That restatement is what gives the model something clean to capture.

"We can start once security approves it" is a useful statement. "The customer agreed to start" is a materially different statement.

The second version removes the condition. A rep reading it later may send an implementation date. A manager may treat the opportunity as further along than it is. The implementation team may start planning around a commitment the customer never made.

This is why I care more about the content of a meeting note than how polished it sounds. A short note with an obvious unresolved item is easier to work with than a confident summary that hides the uncertainty.

Meeting-summary research has documented several error categories, including omissions. Individual reviews of meeting assistants also describe summaries that lose useful nuance. Neither source establishes that any named vendor always behaves this way, but both support checking the substance of the output rather than its fluency. 1 2

Define what belongs in a decision record

For a sales meeting, separate five kinds of information.

Customer context explains the problem, current process and constraints. It helps someone understand why the conversation happened.

Confirmed decisions state what the participants actually settled, including the scope and conditions. A preference is not automatically a decision.

Commitments state who agreed to do what. They may support a decision without resolving it. Sending a security document does not mean security approved the product.

Open questions preserve missing information that matters for the next step. They should be visible enough that someone can take ownership of resolving them.

Proposals capture suggestions that have not been accepted. Keeping them separate prevents an attractive next step from acquiring an owner and deadline simply because the note needs to look complete.

This structure gives the reader a way to distinguish facts before acting on them. It also creates a clear review standard for the person checking the note.

The mistakes to look for

A condition disappears

Fictional source statement:

We can run the pilot if procurement approves the agreement.

Useful record:

Pilot is conditional on procurement approval. Agreement not yet approved.

The condition belongs beside the proposed action. Putting it in a separate paragraph makes it easier for the action to be copied without the qualification.

An individual opinion becomes a company decision

Fictional source statement:

I prefer this workflow, but our regional leads need to review it.

Useful record:

Speaker expressed a preference. Regional review remains outstanding.

Do not label that person a decision-maker or champion based on one positive statement. Those roles need their own evidence.

A suggestion acquires an owner

Fictional source statement:

Someone should send the technical team a sample.

Useful record:

Proposed: send a sample to the technical team. Owner and due date not stated.

The right next action is to establish ownership, not to invent it.

An earlier decision survives a later change

Fictional sequence:

Early in the call: Let's include every location in the pilot.

Later: Actually, begin with two locations and review the results before expanding.

Useful record:

Current agreed scope: two locations. Broader rollout deferred until results are reviewed.

If the conversation contains a genuine unresolved disagreement, preserve it. Do not use the last sentence as a universal rule for choosing the truth.

A copyable decision-ready template

This template is an original proposed format. Test it with your meeting types and review the output before relying on it.

Create a decision-ready sales meeting note from the available meeting content.

Use only supported information. Do not fill gaps with plausible details.

1. Customer context
State the customer's goal, current process and relevant constraints.
Preserve numbers only when their meaning and units are clear.

2. Confirmed decisions
For each decision, include:
- The decision.
- Its scope.
- Conditions or approvals still required.
- Whether the decision changed during the meeting.

3. Commitments
Use a table with:
Action | Owner | Due date | Dependency | Status

Use "Not stated" when an owner or date is absent.
A suggested action is not a confirmed commitment.
Do not invent a date from urgency or enthusiasm.

4. Open questions
List unresolved questions that affect the next step.
Separate missing information from a disagreement between participants.

5. Proposals awaiting confirmation
List suggested next steps that were not accepted or assigned.

6. Items to verify before use
Identify ambiguous names, amounts, dates, ownership or commitments.
Do not turn an uncertain detail into a confident statement.

Keep the note concise. Preserve qualifications that change the meaning.
Use exact quotations only when the original wording is available.
Do not invent timestamps, source links or speaker identities.

The template specifies an output contract. It does not make the model incapable of making mistakes. Its value is that the resulting fields are easier to inspect and reuse.

A complete fictional example

Suppose a buyer says their team spends too much time reconciling weekly reports. They want to test whether a new workflow handles location-level approvals. They cannot share production data until legal approves the agreement.

The seller agrees to send a sample using fictional data. Jordan, the buyer's operations manager, agrees to circulate it to two regional leads. They discuss a technical meeting but do not choose a date.

A useful note would look like this:

Customer context

The buyer wants to reduce weekly report reconciliation. The evaluation needs to establish whether location-level approvals fit the current operating process.

Confirmed decisions

The initial review will use fictional data. Production data will not be shared before legal approval.

Commitments

ActionOwnerDue dateDependencyStatus
Send a sample using fictional data.Seller.Not stated.None stated.Confirmed.
Circulate the sample to two regional leads.Jordan, operations manager. Not stated.Receipt of the sample.Confirmed.

Open questions

Which approval scenarios must the sample demonstrate? Who owns legal review? What feedback will the regional leads provide before a technical meeting is useful?

Proposals awaiting confirmation

Hold a technical review after the sample is evaluated. Participants and date are not agreed.

This note does not claim that the buyer approved a pilot, committed to purchase or selected a rollout date. It gives the seller enough information to coordinate the next step without overstating the opportunity.

Save the structure in Magic Screen

Magic Screen allows you to create a named note template and reuse its instructions. For a meeting-specific decision record, use a snapshot template rather than a Living Note. Attach it to the relevant meeting and review the generated result afterward. 3 4

Name it for the job, such as "Discovery decisions" or "Technical review decisions." A descriptive name is more useful to a rep than "Prompt v7."

Keep the initial format consistent across the team. If every rep changes the same field, find out why. The answer may be a missing instruction, an unnecessary field or a meeting type that needs its own template.

When you edit a saved template, Magic Screen's documentation says the new instructions apply to future output rather than automatically rewriting old generated notes. Treat a template revision as a change to the process, and decide whether older important notes need separate review. 3

Make the last minute of the call help the note

You can reduce ambiguity before the assistant generates anything.

Read the important next steps back to the participants. Ask who owns each one and what the dependency is. Confirm whether a date is a target or a commitment. If something remains unresolved, say so plainly.

For example:

I will send the sample. Jordan will circulate it once it arrives. We have not scheduled the technical review yet. Is that the right sequence?

This gives the participants a chance to correct the record. It also creates clearer source material for any note-taking method.

Do not manufacture agreement to make the meeting feel productive. An honest unresolved item is useful. A false agreement creates downstream work.

Review the fields that drive action

Before sending the note or updating another system, check the decision, owner, date and condition together. A correct action assigned to the wrong person is still an incorrect action item.

When a field is ambiguous, return to the source material available under your approved process. If it cannot be resolved there, ask the participant. Do not ask the model to become more confident about information that was never provided.

Once reviewed, use the decision record to prepare the customer-facing follow-up. Keep the internal questions and customer commitments distinct. The result should make the next conversation easier to run, without requiring someone to reconstruct what the previous one actually decided.

Frequently asked questions

Why do AI summaries miss decisions?

The decisive sentence is usually short and linguistically unremarkable, while the discussion around it is long and detailed. Summarisers weight the discussion and drop the conclusion.

What should a decision record contain?

The decision, the owner, the date, any condition attached, and what it supersedes. The condition is the field most often lost, which turns a conditional agreement into a commitment nobody made.

How do I make decisions easier for AI to capture?

Restate each decision aloud in the last minute of the call. That restatement is clean, explicit and easy to extract, and it also confirms the agreement with the other side.
